A campus-wide site license for the Qualtrics online survey software tool is available to all students, faculty, and staff at Dartmouth. Qualtrics is integrated with Dartmouth’s NetID authentication system (single sign-on) but also allows the sharing of surveys with non-Dartmouth collaborators.
To access the Qualtrics platform, navigate to:
https://dartmouth.qualtrics.com/ (or https://qualtrics.dartmouth.edu - after the initial log in)
and log-in with your NetID and password.
If it is your first time logging in, you will be asked whether or not you have an existing account: always say "NO". If you need to transfer surveys from an existing account (personal or institutional), please contact Qualtrics Support (then select SSO and then Organization ID = "dartmouth", then "Continue") and request the transfer. Your account will be automatically provisioned the first time you log in. Qualtrics is supported on Edge, Firefox, Safari, and Chrome.

- survey translation capabilities, allowing respondents to select a language or default to a browser language preference and tracking those selections in survey results.

Appropriate Use
Human Subjects Research Must Be Reviewed by the IRB. In accordance with federal regulations, research involving human subjects must be reviewed by The Committee for the Protection of Human Subjects (CPHS) which is the Institutional Review Board (IRB) at Dartmouth. Please visit the CPHS website for further information. It is your responsibility to ensure compliance with both federal and Dartmouth research regulations and policies.
FERPA. Qualtrics states that “customers own and control their data” so Dartmouth faculty, staff, and students are responsible for adhering to FERPA regulations.
Per Qualtrics’ Security White paper: FERPA regulations require a state or local educational authority to use “reasonable methods” to ensure “to the greatest extent practicable” that any entity designated as its authorized representative to receive data to conduct evaluations, audits, or compliance activities (1) uses student data only for authorized evaluation, audit, or other compliance purposes; (2) protects the data from further disclosure or other uses; and (3) destroys the data when no longer needed for the authorized purpose.
HIPAA. Surveys created with Dartmouth’s Qualtrics license are HIPAA-compliant.
Qualtrics is designated as a Business Associate (BA) when an agreement is signed with a Covered Entity—an organization that is required to comply with HIPAA privacy rules.

Protecting Privacy All surveys must protect the privacy and confidentiality of student, employee and other institutional information, as required by FERPA (privacy of student information), appropriate state laws, and Dartmouth policies. Although a closed group of participants may complete Qualtrics surveys without the individuals explicitly having to identify themselves, the Qualitrics software stores identifying information in its databases. Consequently, Qualtrics can not be used with absolute guarantees of anonymity for your participants.
Compliance with all other Laws ad Policies Surveys must comply with all other applicable College policies and state and federal laws, including the federal [http://www.copyright.gov/help/faq] laws for copyright and privacy.
The survey service may not be used for personal or commercial gain
Unsolicited mass communication may not be delivered in conjunction with the survey service
Survey participants should be informed that their survey data will be stored on a third party server and that, while the third party has agreed to keep the data confidential, Dartmouth does not control the security of that server.
The Office of the CIO reserves the right to change, at any time, and at its sole discretion, this survey service and these Terms of Use. Notification of changes will be published at rc.dartmouth.edu.

Terms of Use
The terms of use of the site license are:
- The Dartmouth license with Qualtrics is for academic (not-for-profit) use only. Dartmouth and individuals authorized by Dartmouth to use the Qualtrics software may do so for academic teaching or research purposes only. Unless specifically authorized by Qualtrics, Dartmouth and such individuals may not use the software for commercial use nor may any of them assign any rights to use the software to a commercial entity or otherwise permit a commercial entity to use it.
- A subcontractor business associate form may be required. Where the Qualtrics software is being used in connection with a matter where Dartmouth or any faculty, any other employee or any student is acting as a "business associate" to a "covered entity" (as those terms are defined under the rules promulgated under the Health Insurance Portability and Accountability Act of 1996, as amended, and the Health Information Technology for Economic and Clinical Health Act, Title XIII of Division A of the American Recovery and Reinvestment Act of 2009, known as HIPAA and HITECH, respectively), Qualtrics is required to enter into a subcontractor business associate agreement. Dartmouth has pre-negotiated a form of subcontractor business associate agreement with Qualtrics. Contact the Office of Procurement Services or the Office of General Counsel for assistance in putting in place a subcontractor business associate agreement with Qualtrics.
- As soon as possible, please delete all PHI data. In the case of matters described in item 2 above, you are required to delete, de-identify or anonymize all data housed with Qualtrics when such data is no longer needed.
