Create Collaborative SharePoint Forms

This article covers how to create a Microsoft Form that is tied to a SharePoint Team so your whole team can collaborate on the form and see the results together. It also covers where responses are stored, how to find a group form later from forms.office.com, and how to add a shortcut to the form inside your Team's SharePoint site.

Create a New Form for Your Team

Creating the form directly inside your Team's Microsoft 365 Group makes every team member an owner right away, so the whole team can edit the form and view results without any extra sharing steps.

  1. Go to forms.office.com and sign in with your Dartmouth credentials.
  2. On the Forms home page, scroll down to My Groups and select the Team you want the form tied to.
  3. Select New Form.
  4. Add your title, questions, and any other details. Forms saves your work automatically as you go.

Already have a form under your own account? You can move it to your Team.

Moving Individual Form to Team

  1. Open it in forms.office.com
  2. Select the three dot menu on the form
  3. Select Move to a Group
  4. Choose your Team

This turns your personal form into a group form the whole team can access.

Where Your Form Results Are Stored

When a form belongs to a group, Microsoft Forms automatically creates an Excel workbook with all the responses and stores it on that Team's SharePoint site, under Documents > Apps > Microsoft Forms > [your form name]. Any files submitted through the form land in that same folder. Because the workbook lives on the Team's SharePoint site, any team member with access to the site can open it directly, in addition to viewing responses inside the Forms app itself.

Find Your Group Form from forms.office.com

  1. Go to forms.office.com and sign in with your Dartmouth credentials.
  2. Select Shared with Me, then select My Groups.
  3. Select your Team, then select the form you want to open. From here you can review responses, edit questions, or copy the form link to share again.

Add a Shortcut to the Form in SharePoint

Adding a shortcut to your Team's SharePoint document library gives everyone a quick, visible link to the form or its results workbook without needing to search for it in forms.office.com.

  1. In Forms, open your form, select Share, then select Copy to copy the form link. You can also copy the link to the results workbook from SharePoint instead if that is what you want the shortcut to point to.
  2. Go to your Team's SharePoint site and open the document library where you want the shortcut to appear.
  3. Select New, then select URL Shortcut. Depending on your SharePoint version this option may appear as Link.
  4. Paste the link, give the shortcut a clear name so your team knows what it points to, then select Create.
